Purpose: 'Temp. Business/Pleasure Visitor' is wrong for H-1B — select H-1B (H1B). 'Person paying for trip' is usually your employer for H-1B. Address in US = your worksite or residence.

Travel Companions / Previous US Travel

List ALL prior US visits — they have entry/exit records. Include prior visa numbers and any refusals; an undisclosed refusal is treated as misrepresentation.
