Completed and signed by authorized employer representative.

H Classification Supplement to Form I-129

Labor Condition Application (LCA), certified by DOL

Must be certified before I-129 is filed. Valid for the period of H1B employment.

Employer support / offer letter

Details job title, duties, salary, work location, and specialty occupation.

Copy of beneficiary passport (biographical page)

Must be valid for at least 6 months beyond the requested stay.

Copy of all prior visa stamps and I-94 (if in US)

Download I-94 from cbp.dhs.gov

Academic degree certificates + transcripts

Official transcripts. Foreign degrees need NACES credential evaluation.

Resume / CV

USCIS filing fees (I-129 + ACWIA + Fraud + Asylum)

Separate check/money order for $2,805 payable to US Department of Homeland Security.

Prevailing wage evidence (salary meets LCA level)

Recent pay stubs, offer letter confirming salary at or above LCA wage.
